- Affected part and revision
- State the part number, prior revision, new revision, and the exact drawing or specification affected.
- Change description
- Describe what changed in factual terms: dimension, material, feature, software, specification, or process requirement. Do not use a vague label such as “updated.”
- Authorization and effective date
- Identify the customer or design authority approval, approval date, and effective date or serial/lot break where provided.
Step-by-step completion sequence
- Collect the authorized change notice from the customer or design authority.
- Verify that the submitted parts, drawings, FMEAs, control plan, validation results, and PSW reflect the approved condition.
- Complete the change log with traceable identifiers and revisions.
- Identify every validation result affected by the change and include the required evidence.
- Do not label a change authorized until the required approval exists.
PPAP control point: The customer’s current submission request, customer-specific requirements, and approved forms control. This guide explains a practical way to prepare the record; it does not authorize a substitute format or acceptance criterion.
